Someone from the Community Learning Disability Team - it could be a social worker or health worker - will come to visit you at home. They will talk to you about what you are able to do for yourself and what you need help with. This is called an assessment of need and helps us to decide what help you need.
If you have relatives or carers who help look after you, you can involve them in this too. Your carers can also ask for an assessment of their own needs if they want one.
If you need help from us, we will work out, with you, what levelof support you need. You will be able to receive this in the form of an individual budget. This can be offered as a direct payment, where you manage your own support plan or we can arrange support for you.
