Mental Health Problems - What will my social worker do?

Your social worker will discuss your situation with you and will suggest what help may be available. This is an assessment  of need  under the NHS and Community Care Act which will concentrate on the help you need. If you have relatives or carers, we can involve them in this too.

This assessment will take account of your social and health needs and could result in both social care and health services being provided to you. Our social workers are based in hospitals and Community Mental Health Centres around the county.  This helps us work closely with staff in the Health service.

Although we offer a range of services, not everyone will be eligible for them as we have to provide services to those most in need. We will use your assessment of need alongside eligibility criteria to decide which services you are eligible for.

We will try to offer you as many choices as possible so that you can decide what is best for you. We will then make any necessary arrangements for services or further information you need.

To help our work, we have to keep some records about our work with you. This information will be confidential between Cumbria County Council Adult Social Care and yourself and we will not share it with others without your consent. No one else will be allowed to see this without your permission. You have the right to see your records through our Access to Records Procedure.

We will ask you if some basic information about yourself may be shared with Health staff and others who are working with you. This helps us ensure that the help you receive is properly co-ordinated. This is the Care Programme Approach.
