Mental Health Problems - What is a care plan?

We will give you a copy of your care plan  which includes: 

• details of the services you will receive; 
• the name, address and telephone number of your key 
• worker; 
• emergency telephone numbers; and
• when we will review your care.                  

We may review the help you receive to make sure it continues to meet your needs. Your care plan will tell you when your next review is, although you can ask for one at any time if you feel your circumstances have changed.
