If you would prefer to make a referral by post or fax, a referral form is available for use by professional agencies referring adults to Adult Social Care. You can download the form using the link below.
You can use this form to refer non-urgent matters. You can also use it to confirm in writing urgent matters you have previously referred by phone. If you have an urgent matter to refer, please call:
- Customer Services at your nearest Adult Social Care office during office hours; and
- our Out of Hours Service on 01228 526690 outside office hours.
We hope you will find the form easy to use but please note:
- Reason for referral - We need sufficient detail here to enable us to decide if your referral is appropriate for us to deal with and to decide how urgently we should deal with it.
- Ethnicity - Like most professional agencies, we need ethnicity information so we can monitor equality of access.
- Agreement for the referral - It is important that you seek the consent of the person you are referring. There are only limited circumstances in which we will accept a referral without the individual's consent.
- Learning disability screening tool - You should only complete this third page of the form if you are referring someone who you believe has a learning disability.
Please provide as much detail as possible. If you omit essential information, this may delay our response if we have to get back to you to confirm this.
When you have completed the form, you should fax, post or deliver this to Customer Services at your nearest Adult Social Care office.
