What happens next?

You will be contacted by a member of the Occupational Therapy team.   This will usually be within 2 days of the referral being taken by our Customer Advisor.

The Occupational Therapy team will decide if your needs can be met through one of the following approaches:

  • Telephone call and assessment completed
  • An Appointment with either a Social Care Worker or Occupational Therapist at a centre in your locality which you will be invited to attend.
  • A home visit to determine your needs and appropriate options.

Following an assessment of your presenting needs, we will determine if you are eligible for services under Fair Access to Care criteria.   You will not receive services if your overall needs fall under the ‘Low’ or ‘Moderate’ Fair Access to Care categories. We will aim to complete an assessment of your needs within 28 working days where possible.

If your needs are considered to be “Critical” or “Substantial” according to the Fair Access to Care eligibility criteria, we will arrange agreed services or referral to other agencies to consider your needs.   This will be:

  • simple aids to daily living usually provided by a ‘Prescription’, which you can take to an accredited retailer to collect the items identified most appropriate to meet your needs.
  • specialist equipment and advice
  • minor adaptations and possible Direct Payment options to enable you to arrange your own minor works.
  • Major adaptations which are necessary and appropriate via referral to your local district council for a Disabled Facilities Grant
