Guidelines for the Single Assessment Process in South Cumbria

These guidelines are for health and social care staff in South Cumbria.  They have been jointly developed by Cumbria County Council and Morecambe Bay Primary Care Trust and they have been accompanied by briefings for health and social care staff.

The guidelines are primarily for health and social care staff working with older people.  Staff working with adults with physical disabilities and those with chronic health and care needs are expected to work in a similar way.  Staff working with people with learning disabilities and people with specialist mental health services will have their own guidelines to work to.

The guidelines include:

1  a description of single assessment and how older people will benefit from it;
2  a set of shared values and principles that staff should adopt;
3  flowcharts describing the single assessment process;
4  a description of arrangements for information sharing;
5  a glossary of terms used; and
6  an appendix giving examples of how different agencies are adopting the process.
