Making a Request

The Subject Access Request form can be used to request personal information (information that relates to you - the data subject) under the Data Protection Act 1998.

Please remember to provide your name, address and telephone number and a full description of the information you require.  If you can tell us who the information is held by, for example, department or team, it will help us to locate it more quickly.  We will contact you if we need any further details or clarification.

You will be asked to provide proof of your identity before any information is disclosed.  This is to prevent the unauthorised disclosure of information about data subjects.
